Understanding AMH and Ovarian Reserve

Almost every week, a patient sits across from me holding a lab report, pointing to a single number and asking, "Doctor, is this good or bad?" The number is usually AMH — Anti-Müllerian Hormone — and I understand why it feels so weighty. It arrives on a page as a cold, clinical figure, yet it is meant to say something about one of the most personal parts of your life: your fertility. In this piece, I want to walk you through what AMH actually tells us, what it does not, and how I encourage my patients to hold this information without letting it take over their sense of calm.

What AMH Actually Measures

AMH is produced by the small, developing follicles in your ovaries — the tiny fluid-filled sacs that each contain an immature egg. Think of your ovaries as holding a reserve of these follicles, most dormant, a smaller number active in any given month. AMH is released by the ones currently growing, so a blood sample can give us a general sense of how many follicles are present at that stage. This is why we call it a marker of "ovarian reserve" — not a measure of egg quality, not a countdown clock, and not a verdict on whether you can conceive. It is simply one data point among several that helps us understand the landscape of your ovarian function.

Because AMH is produced somewhat steadily by these follicles, it can be checked on almost any day of your menstrual cycle, which is part of why it has become such a popular first-line test. It is convenient, but convenience should never be confused with completeness.

Why Ovarian Reserve Isn't the Whole Story

I often tell patients that ovarian reserve is about quantity, not quality. A woman with a lower AMH can still have healthy, viable eggs among the ones she has. Conversely, a higher AMH does not guarantee that every egg retrieved will be chromosomally normal or capable of resulting in a pregnancy. Egg quality is influenced by age, genetics, lifestyle, and factors we are still learning about — it cannot be read off a single hormone level.

This is why, in my practice, I never look at AMH in isolation. We consider it alongside a woman's age, her menstrual history, an ultrasound assessment of antral follicle count, and sometimes other hormonal markers. Together, these paint a much more complete and useful picture than any one number could on its own.

What a Lower Number Really Means

If your AMH comes back lower than the typical range for your age, it is natural to feel a flicker of worry. I want to gently reframe that. A lower AMH tells us that the pool of follicles available at this point in time is smaller than average — it does not tell us that pregnancy is impossible, nor does it predict how quickly things will change. Many women with lower ovarian reserve conceive naturally or with modest support. What a lower number does is help us plan thoughtfully: it may mean choosing not to delay a workup, considering treatment timing more proactively, or discussing options like fertility preservation earlier rather than later. Knowledge, in this sense, is protective rather than frightening — it gives us more time to make unhurried, informed choices together.

What a Higher Number Really Means

On the other end, a higher-than-expected AMH is not automatically a marker of "excellent fertility" either. Sometimes it reflects conditions such as polycystic ovaries, where a larger number of small follicles are present but ovulation itself can be irregular. In these situations, the conversation shifts toward understanding ovulatory patterns and cycle regularity, rather than reserve alone. This is a good reminder that every lab value needs context — the same number can mean different things for different women.

How We Use This Information Together

When a patient brings me an AMH report, my first question is never "what should we do about this number," but rather "what does this number mean for you, right now, in the context of your whole story." Are you trying to conceive this year, or are you thinking ahead? Do you have a partner, and what does your combined fertility picture look like? Is your cycle regular? Have you had any prior pelvic surgery or conditions like endometriosis that might independently affect reserve? These questions matter as much as the lab value itself, and they are why I believe fertility assessment works best as an ongoing conversation rather than a one-time test result handed across a desk.
